Dear Parents
Kindly come with your child for an interaction on dates mentioned below along with the following documents.
Kindly note: (only one parent either mother or father).
1. Filled Registration Form (Colour Print) which has been sent by email.
2. One Family Photo Postcard size (5x3).
3. Municipal Birth Certificate of the child.
4. Proof of Residence:
a. Ration card of Family/Electricity Bill/Passport (any 1)
b. Aadhar Card of Father/Mother and Child.
5. Baptism Certificate of the Child – in case of Christians only)
6. Caste Certificate (in the Child’s name) – optional
7. Medical (Fitness) Certificate of the child.
8. Kindly submit all the above documents at the time of interaction
